Job Description

Seven Social Care are looking for a temporary Personal Advisor to work with the 16- 25 team in Milton Keynes. This position is initially for a period of 3 months and is paying from £13.00 - £17.00 per hour (paid to you via an umbrella company).

The role:

Work within the legal framework and leaving care policy and demonstrate the necessary personal qualities are capable of becoming an effective professional with the ability to advocate for the young person so they are supported to reach their potential.

Demonstrate skills in assessing young people's needs and designing relevant interventions so that each care leaver is enabled to make positive transitions.

To provide support, advice and assistance to enable young people to maximise education, training, and accommodation opportunities.

To will ensure young people have the right support, at the right time in order for them to transition into adult and leave care.

Key Responsibilities:

To provide direct work, advice, guidance and practical and emotional support to care leavers, including review of Pathway Plans, support visits, and provide advocacy to assist young person to access accommodation, education, employment or training and appropriate services to their needs.

To develop and deliver the Pathway Plan for young people who are looked after, aged 15 years 9 months, or care leavers in consultation with the young person and all partners. Lead the work undertaken on the preparation of the plan in conjunction with the assigned social worker.

To work in a supportive way to undertake specific tasks with young people to assist in meeting the goals of a Pathway Plan.

To ensure the active involvement of care leavers in their leaving care plan.

To monitor and review the outcomes of the plan.

To consult with care leavers in service planning and support them to develop their skills and confidence to make representations in various forums.

Create networks and partnerships in order to develop and improve services to care leavers in Milton Keynes.

To operate in accordance with the Children Act (Leaving Care), departmental leaving care guidance and administrative procedures.

To keep accurate records on young people and update information systems.

To participate in development of a responsive leaving care services that is sensitive to all individual needs.

To work collaboratively with social workers and participation officer. To ensure that the young person engages and is consulted about participation and the wider services development.

* To promote a greater understanding of leaving care issues within the council, members and the wider community.

Applicant Requirements:

* Qualifications: N/A.

* Experience: 2+ years working within a similar role.

* Compliance: Current DBS and references covering the past 5 years.

* Travel: Clean UK driving license with the ability to commute to and from work independently.

* Hours: 37.5 hours p/w.
