Job Description

Detailed job description and main responsibilities The post holder will partake with the physiological assessment of our service users. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests
Assessing service users holistically. Assessing lifestyles - exercise and activities, eating and drinking, smoking, alcohol and drug use and giving advice on healthy lifestyles and signposting our service users to other services.
The post holder will work alongside the physical health nurse in clinics and community visits but will be participate in team working.
The post holder will work with other team members to ensure the team operates a holistic and multi-disciplinary approach
The post holder will contribute to the CPA process - liaising with the care coordinators in view of contributing towards CPA needs and reviews
Have an awareness of how illness, medication and personality can affect an individual's behaviour and an understanding of significant indicators to report to the care co-ordinator.
The post holder will contribute to assessment of both physical and mental health needs. Focusing on the physical health and wellbeing of ours service users with mental health needs and understanding how the relationship between physical and mental wellbeing
The post holder will arrange physical health assessments with the service user both at home and in clinics.

The Provide information on health promotion including healthy eating, sport and exercise and smoking cessation

The post holder will have understanding/experience of mental illness and will operate in a service user focused and non-judgmental manner.

Person specification Qualifiaction Essential criteria
  • Recognised competency based level 3 NVQ in relevant subject area or equivalent demonstrable experience


Experience Essential criteria
  • Experience of working in MH care setting
  • Experience working in a community setting
  • Experience of MDT working


Skills Essential criteria
  • Excellent written and oral communication skills
  • Good organisational skills and ability to work flexibly

Desirable criteria
  • Understanding of how to engage service users
  • understanding of health promotion
  • Venepuncture trained
  • understanding of physical health needs for MH service users
  • skills in physical healthy care observations
