Seven Social Care is looking for a Social Care Assessor to work within the Countrywide Continuing Healthcare Team for North Yorkshire County Council. These assessors can be Nurses, Occupational Therapists or Social workers.

The role:

As a Social Care Assessor, you will work with Countrywide Continuing Health Care Team in Scarborough Hospital and will be required to work from the hospital daily with an established discharge team.

Key Responsibilities:

· The work is predominately to be a member of the multidisciplinary team to support the completion of the decision support tool and to agree an eligibility decision.

· As the representative from the local authority, you will need to be able to complete care act assessments and to be able to determine what needs may be beyond that which social care can provide.

· There will be a requirement to complete and/or update care act assessments so that we have up to date information and evidence to support the outcomes for the person.

· Experience and knowledge of Continuing Healthcare as well as the Care Act is essential.

· All our team are currently working virtually rather than face to face and the DSTs are completed using MS Teams. On this basis, location can be flexible.

Applicant Requirements:

* Qualifications: Qualified registered Nurse, Occupational Therapist or Social Worker

· Experience: knowledge of Continuing Healthcare as well as the Care Act is essential.

* Compliance: HCPC registration and DBS

* Hours: 37 hours p/w.
