What You Will Do

Work collaboratively with individuals, carers, and partner agencies to deliver integrated and strengths-based assessments

Develop person-centred care and support plans that promote independence and successful outcomes

Manage a diverse caseload and ensure timely responses to emerging needs

Provide accessible information and immediate support services where required

Assess risks to independence using a range of tools and contribute to risk management planning

Support individuals through safe and effective hospital discharge planning

Provide advice on funding options to maximise informed personal choice

Protect individuals' rights under the Human Rights Act and Mental Capacity Act, ensuring least restrictive approaches

Complete financial assessments in line with statutory requirements and maintain accurate financial records

Work alongside a range of providers and organisations to develop creative community-based solutions

Prepare detailed reports for case conferences and safeguarding forums

Contribute to investigations under Protection of Vulnerable Adults procedures
