What You Will Do

  • Work collaboratively with individuals, carers, and partner agencies to deliver integrated and strengths-based assessments

  • Develop person-centred care and support plans that promote independence and successful outcomes

  • Manage a diverse caseload and ensure timely responses to emerging needs

  • Provide accessible information and immediate support services where required

  • Assess risks to independence using a range of tools and contribute to risk management planning

  • Support individuals through safe and effective hospital discharge planning

  • Provide advice on funding options to maximise informed personal choice

  • Protect individuals' rights under the Human Rights Act and Mental Capacity Act, ensuring least restrictive approaches

  • Complete financial assessments in line with statutory requirements and maintain accurate financial records

  • Work alongside a range of providers and organisations to develop creative community-based solutions

  • Prepare detailed reports for case conferences and safeguarding forums

  • Contribute to investigations under Protection of Vulnerable Adults procedures

Salary range
