What you will do

• Undertake Care Act assessments and reviews for adults with learning disabilities.
• Work in a person-centred, strengths-based, and outcome-focused manner.
• Develop, implement, and review personalised care and support plans.
• Promote independence, choice, control, and social inclusion.
• Assess and manage risks while respecting individuals' rights and decision-making abilities.
• Collaborate with individuals, families, carers, advocates, and partner professionals.
• Coordinate comprehensive care packages, incorporating community and commissioned services.
• Exercise professional judgement regarding eligibility, safeguarding, risk management, and support needs.
• Conduct safeguarding enquiries and contribute to safeguarding processes.
• Support individuals through transitions and significant life changes.
• Work closely with health services, housing providers, voluntary organisations, and partner agencies.
• Maintain accurate and timely case records that meet professional and organisational standards.
• Manage a diverse caseload, prioritising work according to assessed need and risk.
• Participate in team meetings, supervision, reflective practice, and professional development activities.
• Contribute to service improvement initiatives and the development of effective support strategies.
