What You Will Do:

Conduct Regulation 24 assessments, viability assessments, and full connected carers assessments in line with statutory requirements.

Provide practical and emotional support to connected carers, ensuring they are fully prepared to meet children's needs.

Develop and implement sustainable care plans in partnership with families and other agencies.

Liaise effectively with schools, health professionals, and partner organisations to deliver cohesive care solutions.

Deliver training and support sessions to connected carers, enhancing their understanding and confidence in their roles.

Maintain accurate records, reports, and documentation in accordance with service and legal standards.
