Job Purpose:
The Social Worker - Children with Disabilities Team will provide person-centred services to children with disabilities, focusing on assessments, care plans, and direct support. You will collaborate with healthcare, education, and community services to create a holistic support network, ensuring that children and their families receive the care they need to thrive.
What You Will Do:

Assess and Identify Needs: Conduct comprehensive assessments for children with disabilities and their families to identify their needs, strengths, and goals.
