Job Purpose:

The Social Worker - Children with Disabilities Team will provide person-centred services to children with disabilities, focusing on assessments, care plans, and direct support. You will collaborate with healthcare, education, and community services to create a holistic support network, ensuring that children and their families receive the care they need to thrive.

What You Will Do:

  • Assess and Identify Needs: Conduct comprehensive assessments for children with disabilities and their families to identify their needs, strengths, and goals.

  • Develop Tailored Care Plans: Create individualized care plans that account for the child's disability, along with any additional health, social, or educational needs.

  • Review and Update Plans: Regularly review care plans to ensure that the child's needs are being met and make adjustments as necessary.
