What you will do:
- Conduct outcomes-focused and strength-based social work assessments and interventions to support adults in the community.
- Manage a diverse caseload independently, ensuring effective use of your time while meeting team responsibilities and demands.
- Engage in meaningful conversations and establish preventative measures to address client needs and promote well-being.
- Collaborate with a range of professionals across health and social care services to provide a coordinated response to the community.
- Participate in a duty rota to address urgent support needs and crisis situations effectively.
- Ensure safeguarding measures are in place and adhered to, in line with the Care Act and local policies, protecting vulnerable adults in the community.
About you:
- You are motivated to challenge and improve existing practices within the service, ensuring higher standards of care.
- Possess a strong commitment to adopting outcomes-focused and strengths-based assessments in your work.
- Have a minimum of three years post-qualifying experience in statutory adult social care.
- You are dedicated to safeguarding the adult population, playing a critical role in protecting those at risk.
- Ready to engage in peer support and willing to leverage the diverse experiences of your team to provide excellent care.
