About the Role:
Based in South Gloucestershire, you'll play a vital role in promoting independence and delivering person-centred social care in line with the Care Act 2014. You'll conduct assessments, develop care plans, and coordinate safeguarding interventions to ensure adults receive the right support at the right time. Collaboration with health, housing, and voluntary partners will be central to achieving positive outcomes.
What You Will Do:

Conduct strength-based assessments that identify personal strengths, networks, and resources.

Develop and review person-centred care plans promoting independence and wellbeing.

Undertake safeguarding enquiries (Section 42) and manage risk in line with statutory frameworks.
