What You'll Be Doing

  • Collaborate with internal teams and external agencies to meet the needs of vulnerable adults.

  • Place service users and carers at the heart of care planning, ensuring their voices are central to decisions.

  • Conduct Mental Capacity Assessments and lead Best Interest decisions.

  • Manage Court of Protection applications and attend hearings when required.

  • Lead on safeguarding enquiries and address cases of self-neglect, ensuring legal and ethical compliance.

  • Promote reablement, independence, and well-being by linking individuals with the right services and support pathways.

What You'll Need

  • SWE registration and a recognised Social Work qualification

  • At least two years' experience in adult social care, ideally within a community setting

  • Knowledge of the Care Act, Mental Capacity Act, and safeguarding legislation

  • Strong assessment, communication, and case management skills

Salary range

  • £32.50 per hour
