This exciting ongoing opportunity is your chance to join the Adult Care Assessment and Triage Team within Derbyshire County Council as a Social Worker specialising in Adult Social Care. What you will do:

  • Conduct professional triage and assessments of Adult Social Care needs at the front door of Derbyshire Adult Care services.
  • Respond effectively to adults presenting with a variety of needs, disabilities, and complex circumstances.
  • Deliver person-centred and strengths-based assessments remotely using a range of online communication tools.
  • Apply legislation including the Care Act 2014, Making Safeguarding Personal principles, and the Mental Capacity Act appropriately within practice.
  • Work collaboratively with colleagues, partner agencies, and multidisciplinary teams to achieve positive outcomes.
  • Manage workload independently while maintaining a proactive and solution-focused approach.
  • Maintain accurate and timely case records in line with statutory and organisational requirements.

Requirements:

  • Qualified Social Worker with relevant Social Work England registration.
  • Experience within Adult Social Care is essential.
  • Strong understanding of the Care Act 2014, safeguarding, and the Mental Capacity Act.
  • Ability to work independently and within a team environment.
  • Excellent communication, assessment, and problem-solving skills.

Salary range

  • £35 per hour
