Join Our Team and Make a Real Difference Every Day

Are you passionate about supporting adults to live fulfilling, independent lives? Do you want to be part of a friendly, experienced team where your skills are valued, your wellbeing matters, and your development is actively supported?

If so, we'd love to hear from you.

At Derbyshire County Council, an exciting opportunity has arisen to join our Chesterfield Long Term Adult Social Care Team, as a Social Worker. You'll become part of a well-established, highly supportive team of Social Workers and Social Care Practitioners who are committed to delivering outstanding person-centred practice and achieving positive outcomes for the people we support.

Why Choose Chesterfield Long Term Adult Care?

Every day brings new opportunities to make a meaningful impact. Our team supports adults with a wide range of care and support needs, including young people transitioning into adulthood, individuals living within their communities, and those residing in care settings.

Our practice is firmly grounded in the principles of the Care Act 2014, with a strong emphasis on strength-based and person-centred approaches, empowering people to maximise their independence, wellbeing, choice and control.

What You'll Be Doing

As part of our team, you'll have the opportunity to:

Build positive, meaningful relationships with adults and their carers through person-centred assessments that focus on strengths, aspirations and outcomes.

Lead personalised safeguarding enquiries, using your communication, analytical and organisational skills to support people at times when they need us most.

Work collaboratively with health colleagues, providers, community organisations and partner agencies, playing a key role in effective multi-disciplinary working.

Support individuals to make informed choices and achieve greater independence, ensuring their voices remain central to decision-making.

Develop strong relationships with local care homes through our dedicated Link Worker role, undertaking regular visits and monthly drop-in sessions that foster partnership working and strengthen outcomes for residents and providers alike.

We believe that supporting our workforce is just as important as supporting the people we work with.

✨ Enjoy a genuinely friendly, welcoming and supportive team culture where colleagues look out for one another and celebrate success together.

✨ Benefit from regular contact and support through daily team catch-ups, accessible managers and senior practitioners, monthly supervision sessions and weekly face-to-face pod meetings.
