What You Will Do:

• Supervise and manage a team of four Social Care Practitioners and one administrator.

• Sign off and oversee Care Act Reviews and assessments for older adults, and individuals with learning disabilities and autism.

• Support staff with mental capacity assessments, providing guidance and case direction.

• Build connections with third sector organisations to enhance outcomes for service users.

• Lead a one-day-a-week duty rota for the Learning Disability Service, managing new referrals, package queries, and safeguarding triage.

• Drive targeted project work to improve reviewing outcomes across key service user groups.
