Social Care Practitioner Opportunity, Chesterfield START Team

Fixed-term maternity cover until June 2027

We are looking for an enthusiastic and motivated Social Care Practitioner to join our Short Term Assessment and Reablement Team (START) in Chesterfield, providing maternity cover until June 2027.

START is a busy, supportive and forward-thinking service working with people in their own homes to maximise independence, promote recovery and reduce the need for longer-term care and support.

What will you be doing?

As a Social Care Practitioner within START, you will:

• Complete strengths-based assessments and reviews, focusing on what people can do and what matters to them.
• Work with individuals, families and carers to identify achievable outcomes that promote independence.
• Support people through periods of short-term assessment and reablement, including those returning home following a hospital stay.
• Work closely with colleagues across our multidisciplinary team, contributing to timely decision-making and effective support planning.
• Identify opportunities for reablement, step-down and community-based solutions, helping people achieve maximum independence.
• Work collaboratively with health professionals and other Adult Social Care colleagues.
• Ensure the person's voice, choices and outcomes remain central to assessment and support planning.
