This is an exciting opportunity for an experienced Social Worker to join the Preparing for Adulthood Team, supporting young people aged 14 to 25 as they transition from Children's to Adult services.
This role offers a unique opportunity to step away from a traditional Adults caseload and work across both Children's and Adults' legislation. You will play a key role in shaping a developing Preparation for Adulthood service, focusing on transition planning, EHCPs, SEND, safeguarding, Mental Capacity Act assessments, Continuing Healthcare, and strengths-based support planning. Available on either a part-time or full-time basis, this position allows flexibility while helping young people build independence and achieve positive long-term outcomes.

What you will do
- Support young people through their transition from Children's to Adult services.
- Utilise EHCPs, SEND frameworks, and safeguarding procedures in everyday practice.
- Undertake strengths-based assessments and develop person-centred support plans that promote independence.
- Work collaboratively with partner agencies and professionals to deliver coordinated support.
- Apply Children's and Adults' legislation confidently and effectively.
- Support young people with complex needs to achieve positive outcomes and greater independence.
- Contribute to the ongoing development of the Preparing for Adulthood service.
- Undertake safeguarding responsibilities and support risk management processes.
- Apply the Mental Capacity Act and Continuing Healthcare frameworks where appropriate.
- Maintain accurate case records and professional documentation in line with statutory requirements.
