We are recruiting an experienced Senior Mental Health Practitioner to join the Crisis Resolution and Home Treatment (CRHT) Team across Norfolk & Suffolk. Working from Northgate Hospital in Great Yarmouth, you will play a vital role in supporting individuals experiencing acute mental health crises, helping to prevent unnecessary hospital admissions and promoting recovery within the community.

Key Responsibilities

• Conduct comprehensive crisis assessments and provide effective gatekeeping to prevent unnecessary hospital admissions.

• Deliver home treatment interventions, including medication support and monitoring concordance.

• Carry out home visits, offering timely and responsive care in line with CRHT pathways.

• Collaborate closely with the multidisciplinary team, contributing to care planning and risk management.

• Provide expert mental health support and guidance to service users and their families or carer
