Band 6 Senior Community Mental Health Practitioner (CPN) wanted in Harlow, UK. Earn £27 hourly for a rewarding full-time role, working 37.5 hours per week on an ongoing basis.
Join us as a key player in our Community Transformation Programme. As a Registered Mental Health Nurse with our established Multidisciplinary Team, you will provide recovery-focused treatment and support to individuals with a variety of mental health disorders. Guide them from diagnosis through recovery and empower them to live the lives they desire. Work alongside a spectrum of clinical professionals, including Nurses, Psychologists, and Support Time Recovery workers, as well as a range of collaborative system partners. This is an exciting opportunity for a positive team player interested in working compassionately with individuals and their support networks.

What you will do:
- Provide and oversee the management and care of service users.
- Produce accurate and timely documentation, maintaining records in line with your professional Code of Conduct and Trust policy.
- Undertake comprehensive assessments and formulate personalised care plans that reflect the needs of service users and their support networks.
- Conduct risk assessments and manage potential hazards in service users' environments.
- Attend weekly Multidisciplinary Team meetings to collaborate on patient care.
- Report and document incidents as required.
- Conduct and oversee physical health care checks and act upon findings.
- Act as Care Co-ordinator/Lead Practitioner, handling responsibilities surrounding medications and psycho-social interventions.
- Offer practical support and education to service users and carers to prevent relapse and potential hospital admissions.
Requirements of the Community Psychiatric Nurse:
- Degree or equivalent in Mental Health Nursing.
- Proven experience working as a Mental Health Nurse.
- Current NMC Registration.
