What You Will Do:

  • Carry out assessments to determine service needs, identify risks, and ensure appropriate interventions.

  • Work collaboratively with health professionals, agencies, and partner organisations to deliver joined-up support.

  • Manage an allocated caseload in line with service priorities and statutory responsibilities.

  • Maintain accurate, timely records and complete all required documentation and administrative tasks.

  • Participate in supervision sessions, team meetings, and training to support your ongoing development.

  • Contribute to service evaluation, continuous improvement, and emerging initiatives.

  • Provide supervision and guidance to social work students where required.

  • Work across various locations within the Borough to deliver effective face-to-face support.

Salary range
