What you will do:

  • Build meaningful connections as you identify and engage with rough sleepers, offering essential support to service users across Brighton.
  • Act as the main care coordinator for specific service users, conducting screenings, asset mapping, and recovery planning, using motivational interviewing techniques.
  • Coordinate and implement an individualised care package including psycho-social interventions, one-to-one key work, and group sessions.
  • Complete comprehensive service user assessments, including mapping community resources, and develop recovery plans.
  • Support and develop service-based recovery communities, fostering positive relationships within the community.
  • Perform risk assessments and manage risk, ensuring the safety and well-being of service users.
  • Triage assessments and make appropriate referrals to a range of treatment and community support agencies.
