What you will do:

  • Identify and engage with rough sleepers and beggars through structured street outreach shifts at various times and locations, following established protocols

  • Act as the main care coordinator for allocated service users, delivering screening, asset mapping, comprehensive assessments, recovery planning, and care reviews using motivational interviewing techniques

  • Coordinate tailored care packages including psycho-social interventions, one-to-one key work, POD and group work, clinical interventions, harm minimisation, and brief interventions

  • Complete comprehensive service user assessments and develop individual recovery plans focused on substance use reduction and community reintegration

  • Support the development and sustainability of service-based recovery communities

  • Undertake risk assessments and manage identified risks effectively

  • Conduct triage assessments and facilitate referrals to appropriate treatment services and community support agencies

Salary range
