What You Will Do:

  • Manage a caseload of young people aged 16-17, supporting their transition to independence.

  • Create Pathway Plans collaboratively with young people, setting goals and mapping out their journey to adulthood.

  • Work closely with wraparound services including Staying Close, Peer Mentoring, and emotional wellbeing support to provide comprehensive care.

  • Collaborate with care leaver and children in care services to ensure a seamless and supportive transition.

  • Build positive relationships with young people, empowering them in their decision-making processes.

Salary range
