Location: Heywood, OL10 1NG

Salary: From £27,000 + bonus & on-call (DOE)

Hours: Full-time | Monday to Friday

About the Role

The role of Care Coordinator is pivotal to the day-to-day running of our busy, established Heywood office. We are specifically looking for someone with proven experience and strong coordination skills, capable of managing complex care arrangements with confidence and professionalism.

Working closely with care staff and clients, you will be responsible for the planning, coordination, and ongoing management of care packages, ensuring seamless service delivery. This is a role for someone who thrives in a fast-paced environment, can work under pressure, make informed decisions quickly, and has the experience to maintain high standards of care.

This role includes an out-of-hours rota due to the nature of complex care, where timely support is essential and situations can be high pressure. There is no requirement to provide hands-on care; the focus is on supporting and managing arrangements outside normal office hours.

You will be based at our Heywood office, with regular travel across the Northwest region. A Full UK Driving Licence and access to your own vehicle are essential, along with confidence driving long distances and on motorways.

Key Responsibilities
  • Act as the first point of contact for clients, families, and healthcare professionals, providing knowledgeable and confident support.
  • Effectively manage rotas for your care teams, using your experience to respond quickly and efficiently to day-to-day changes.
  • Monitor care delivery to ensure it aligns with individual care plans and meets quality and safety standards.
  • Participate in multi-agency meetings with Integrated Care Boards (ICBs), using your expertise to advocate for client needs and contribute to care planning.
  • It is essential that you support the out-of-hours on-call service (no hands-on care required).
What You Will Need

We are looking for a skilled, experienced Care Coordinator who can demonstrate:
  • A Full UK Driving Licence and access to your own vehicle, with confidence travelling across the Northwest region.
  • Proven experience in a Care Coordinator role.
  • Strong experience in scheduling and rota management.
  • A solid understanding of person-centred care and care planning.
  • Excellent organisational skills, with the ability to manage multiple care packages and adapt to changing priorities.
  • Confident decision-making skills, with the ability to stay calm, professional, and solution-focused.
  • Clear and adaptable communication skills, suitable for families, professionals, and external partners.
  • Strong negotiation and influencing skills, particularly when balancing rota and client needs.
  • A proactive, positive approach and the ability to build strong working relationships.
