What you will do
- Provide senior clinical leadership, specialist mental health assessment, and direct care across the Adult CMHT pathway.
- Undertake comprehensive assessments, short-term interventions, ongoing care coordination, complex case management, and discharge planning.
- Support complex formulation, personalised care planning, risk assessment, risk management, and safeguarding processes.
- Implement and promote the use of DIALOG+ and other recovery-focused interventions.
- Manage an agreed caseload, delivering assessment, formulation, care coordination, and profession-specific interventions.
- Provide line management, supervision, leadership, and clinical support to Band 6 practitioners and wider team members.
- Promote a culture of shared multidisciplinary responsibility, therapeutic engagement, and outcome-focused practice.
- Collaborate with service users, carers, partner agencies, and healthcare professionals to deliver high-quality mental health support.
