Salary: £28 hourly - Ongoing Full Time 37.5 hours

What you will do:
- Work as a Care Coordinator as part of a multi-disciplinary team to deliver comprehensive assessments, planning, and review of health and social care needs.
- Provide effective case management to your portfolio of service users in accordance with the Care Programme Approach.
- Ensure effective discharge care planning.
- Promote health and well-being of patients, maintaining a safe and therapeutic environment.
- Undertake risk assessments and formulate comprehensive care plans with patient cooperation.
- Contribute to effective multi-disciplinary teamwork and carry out duties in accordance with professional, CNWL, and Local Authority policies.
- Communicate effectively with internal departments and external agencies regarding client care.
- Offer a range of evidence-based interventions, proactive risk management strategies, and monitor treatment effectiveness.
- Mentor and offer supervisory support to junior staff and nurture professional development.
