What you will do:
- Respond swiftly and effectively to young individuals presenting in crisis, working closely with senior clinical staff.
- Contribute to critical assessments and manage crisis situations using a structured Risk Assessment and Risk Reduction framework.
- Establish therapeutic goals to support young people and their families or carers, ensuring a holistic approach to care.
- Refer to specialised in-patient services when required and collaborate with other services to minimise the need for inpatient care.
- Create and implement comprehensive, safety-focused, recovery-driven care plans to bolster resilience and prevent future crises.
- Participate fully as a key member of the multi-disciplinary team, offering intensive support under various care pathways including rapid intervention and longer-term care plans.
- Engage directly with young individuals in various environments such as emergency care settings, community environments, and home settings.
- Rely on your core qualification as a Mental Health Practitioner to perform bio-psycho-social risk assessments and create safety and care plans.
