What you will do

• Manage the duty function of the Meanwood Team alongside another social worker.
• Maintain and update the duty handover to ensure seamless continuity of service.
• Attend multidisciplinary team (MDT) meetings via Microsoft Teams and contribute to coordinated care planning.
• Manage and update the duty calendar, ensuring visits and interventions are scheduled appropriately.
• Provide advice, guidance, and support to adults requiring social care intervention.
• Undertake assessments and contribute to decision-making processes in line with Adult Social Care legislation and best practice.
• Respond promptly to referrals, enquiries, and urgent situations as part of the Duty Team function.
• Work collaboratively with health professionals, partner agencies, and community organisations to provide effective support.
