What you will do

  • Manage a varied caseload, ensuring individuals receive personalised and outcome-focused support.
  • Undertake Care Act Assessments and reviews in accordance with relevant legislation and best practice.
  • Develop and review strengths-based Support Plans that promote independence and wellbeing.
  • Complete Mental Capacity Assessments and Best Interest Decisions where required.
  • Signpost individuals to appropriate internal and external services to maximise support and community engagement.
  • Build effective working relationships with health professionals, partner agencies, and community organisations.
  • Support adults to remain independent and safe within their own homes and communities.
  • Maintain accurate, timely, and comprehensive case records and documentation.
  • Participate in service development initiatives and contribute to the ongoing transformation of Adult Social Care services.
