What you will do

• Provide essential statutory social work services to vulnerable adults and their carers, following local and national policy guidelines.
• Collaborate in multi-disciplinary team settings to deliver integrated care solutions effectively.
• Safeguard adults through managing risks, documenting, and sharing vital information with partners.
• Develop and execute person-centred care and support plans, using a strengths-based approach to meet individual needs.
• Contribute to the development of the team by participating in activities and providing support under the direction of the Team Manager or Advanced Practitioner.

As a member of Salford City Council’s Adult Social Care and Health Partnerships, you will be at the forefront of delivering critical social work services in mental health. We welcome registered social workers who boast previous experience with adults and demonstrate a strong understanding of relevant legislation, including the Care Act and Mental Health Act.
