What you will do
- Conduct comprehensive Care Act assessments and reviews of adults' needs.
- Identify strengths, abilities, and resources while supporting individuals to achieve their desired outcomes.
- Develop tailored care and support plans that promote independence, wellbeing, and choice.
- Manage risks and respond appropriately to safeguarding concerns.
- Participate in safeguarding adults enquiries where required.
- Use professional judgement to determine eligibility and identify suitable support options.
- Work collaboratively with individuals, families, carers, advocates, and partner professionals.
- Coordinate care packages using commissioned services and community resources.
- Support adults with complex or changing needs.
- Consider Mental Capacity Act principles and undertake best-interest decision-making where required.
- Work effectively with health, housing, voluntary sector, and partner organisations.
- Maintain accurate, timely, and comprehensive case records.
- Manage a varied caseload, prioritising work according to risk and urgency.
- Participate in supervision, team meetings, case discussions, and reflective practice.
- Contribute to service improvement initiatives within Adult Social Care.
- Ensure compliance with legislation, professional standards, council policies, and safeguarding requirements.
