What you will do

  • Caseload Management: Manage a complex caseload of adults with significant care and support needs.
  • Assessment & Planning: Conduct strengths-based assessments and develop person-centred care plans.
  • Advanced Practice: Lead on complex cases, offering specialist advice and professional consultation.
  • Legislation Implementation: Ensure compliance with the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks.
  • Leadership & Mentoring: Provide guidance, mentoring, and professional leadership to colleagues.
  • Reflective Practice: Facilitate case discussions and practice reviews to improve team performance.
  • Safeguarding: Lead safeguarding enquiries and investigations, ensuring safe and effective outcomes.
  • Partnership Working: Collaborate with health, housing, and voluntary sector partners.
  • Quality Assurance: Contribute to service improvement and quality assurance processes.
