What You Will Do:

• Manage a caseload efficiently whilst providing support and guidance to adults living at home.
• Conduct Care Act Assessments, assemble Support Plans, and engage in Mental Capacity Assessments and Best Interest Decisions.
• Signpost individuals to internal and external services as necessary to promote wellbeing and independence.
• Forge effective professional relationships with multidisciplinary teams and local health and social care services for a coordinated support strategy.
• Communicate expertly, ensuring information is clear and correct to better support individuals and families.
