What You Will Do:

• Review Section 117 Aftercare eligibility in line with the Mental Health Act
• Complete Statement of Need (SoN) reviews to ensure appropriate care provision
• Undertake Care Act assessments and develop support plans
• Collaborate with NHS colleagues and multi-agency partners to deliver integrated care
• Participate in multidisciplinary meetings and contribute to care planning

Requirements:

• Strong knowledge of the Mental Health Act and Care Act
• Experience working within adult social care and multi-agency environments
