• Manage and process a backlog of new adult social care referrals efficiently.

• Identify opportunities for reablement and community-based support to encourage independence.

• Conduct Care Act Assessments to assess eligible needs and determine appropriate services.

• Conclude cases promptly, transferring long-term needs to relevant teams when necessary.

• Work autonomously to handle weekly allocations and ensure timely progress on caseloads.
