We are seeking an experienced Adult Social Worker to join the Community Mental Health Team in the vibrant city of Liverpool. What you will do

  • Manage a varied caseload of adults receiving support from secondary mental health services, delivering person-centred interventions and ongoing case management.
  • Complete strengths-based assessments, develop care and support plans, and regularly review outcomes to promote independence and recovery.
  • Work closely with the Transition Team, supporting young people moving into adult mental health services. Experience within Children's Social Care is desirable but not essential.
  • Collaborate with service users, families, healthcare professionals, and partner agencies to provide coordinated, holistic care.
  • Maintain accurate, timely, and legally compliant case records and documentation in line with statutory requirements and professional standards.

Requirements

  • Qualified Social Worker with current Social Work England registration.
  • Experience working within Adult Social Care, ideally in a Community Mental Health Team or similar setting.
  • Strong understanding of the Care Act 2014, Mental Health Act, Mental Capacity Act, and related legislation.
  • Excellent assessment, care planning, communication, and multidisciplinary working skills.
