We are currently recruiting for a Community Mental Health Team (CMHT) Social Worker in a south west England based NHS trust .

The Role You will be working within a multi disciplinary team carrying out assessments and planning the individual packages of care for service users and carers & acting as a care coordinator as required in line with the trust care. As part of the role you will be asked to promote the independence and well-being of adults. A key part of the role is to provide overall responsibility for managing individuals (aged 18 to 65 years) through rehabilitation into the community & assessing individual risk, promoting engagement with multi agencies, interventions care planning & case management

Essential Skills The ideal candidate will be an experienced adult community mental health social worker, with experience of case management and risk assessment planning. Ideally they will have previously implemented appropriate interventions with partnering agencies. Experience in mental health, challenging and social behaviors, therapeutic relationship building with clients, risk assessing and implementing of care co-ordination, court report writing and maintaining comprehensive records to high standards is all desirable.

You must be a qualified BA/MA social worker / Dip SW or equivalent, be professionally registered with the HCPC and have 1 year+ post qualified experience within community mental health or forensic. You must be a car driver.
