An exciting opportunity has arisen with Hampshire County Council for a Qualified Social Worker within the Hospital Discharge Team based at Southampton General Hospital.
You will join a team committed to safeguarding and promoting the welfare of adults. You will be responsible for leading the assessment, support planning and review process for new referrals. You will be working closely with statutory agencies, voluntary and private sector agencies, carers and those with responsibility to ensure you can be a real part of the borough and work alongside all agencies to get the best result for the adult involved.
You will be Working with older adults and adults with physical disabilities in the acute hospital setting including adults of all ages with physical disabilities and adults over the age of 65 with mental health disabilities (some work based at the mental health unit in the hospital with people subject to sections under the Mental Health Act including Section 3).
Ideal candidate will be experienced in:
- Assessment/review (including community reviews shortly after discharge), and care planning
- Mental capacity assessments
- Best Interests processes
- Working with people who are subject to a deprivation of liberty safeguard
- Safeguarding processes
- Continuing Health Care work/liaison
- Carers' assessments
The Hospital Team is currently contributing to and reviewing the development of ongoing initiatives to achieve a more effective use of resources within Adult services. Coming from an adult services background and having good knowledge of all relevant legislation along with HCPC registration will ensure your success. This is a fantastic opportunity for you to really make a difference.
The majority of the work is based at the Hospital but Devon do work with residents of the South Devon area, so any workers would need to be able to travel with in the area/requirements of the job to complete reviews etc.
